The California Wellness Foundation (TCWF) commissioned outside consultants to conduct a confidential survey of TCWF grantees and those who were declined funding during 2009. The evaluators’ report, survey results and the survey questionnaire can be accessed above.
This is the fifth Grants Program Survey that TCWF has commissioned since 1997. The survey is part of the Foundation's commitment to learning. Survey feedback is used to enhance TCWF's responsiveness to organizations that seek funding.
The Foundation has had many inquiries for information on the survey. In an effort to give people a better understanding of the type of information collected, TCWF has made the questionnaire available in PDF format. This survey is for informational purposes only. Although TCWF appreciates feedback, please do not complete and mail this form.
